Company/Organization requesting donation. (included website if available)
Event the donation is being requested for.
Date the donation must be received.
Type of hunt/activity being requested.
Is the donation being given through an auction/drawing/contest etc?
Contact person for company/organization making request.
Short summary of how the donation will benefit the company/organization and how the proceeds from the donation will be used.
